Once your medical professional has diagnosed your condition, you can begin the most effective treatment that is appropriate for your tendinitis. There are a variety of options available depending on the severity of your injury. The good news is that most cases of tendonitis will heal with simple home conservative treatments. It's important to treat tendon injuries as soon as possible. Any activity or strain you put on your tendon can cause further damage to your weakened tissue. Though the pain, discomfort, and inconvenience of a tendon injury can be overwhelming, it's possible to overcome it with consistent treatment on a daily basis.


Start treatment of tendonitis as soon as you are injured

Doctors, PTs and medical professionals all agree that the trick to any tendon injury is getting it to heal with minimal scar tissue formation and with as much realignment of tendon fibres as possible.

Even with optimum healing, there's always less elasticity in a previously injured tendon. This will cause the tendon to hurt during exercise and everyday activities. However, if you treat your soft tissue injury efficiently and quickly with conservative treatment methods, your chance of re-injury later on is much lower.

Most doctors will recommend conservative treatment options to heal the torn tendon tissues as a minimally invasive approach. Other approaches (like surgery) are known to introduce even more scar tissue into your injury, and so this is why doctors will always recommend conservative treatment methods first as a "try everything" approach. The more diligent you are with your treatment and rehabilitation, the faster you will see successful results.

It is generally understood by doctors and surgeons, that surgery will introduce more scar tissue into the tendon and surrounding soft tissue. This added scar tissue will be problematic, requiring PT and conservative treatment options post-surgery. If not dealt with properly, you could end up in worse condition than before the surgery. This is why surgery is only performed as a last resort.

Most doctors, physicians and orthopedic specialists will recommend conservative treatment for minor tendon and muscles injuries before considering surgery.

Some conservative treatment methods recommended for tendonitis include:

  • Rest - This is important for initial healing to reduce pain, swelling and inflammation in the early stage of your soft tissue injury. Too much rest can also be harmful as joint immobility can actually cause stiffening, overcompensation and atrophy (wasting away of soft tissue). This is why rest should be used when reducing initial pain and swelling, but should not be considered for more long-term conservative treatment.
  • Avoid Activity that caused your injury. - While resting the area, it's also important to avoid all activities that may have caused your symptoms - including any repetitive movements (possibly work or hobby related). This may include reduced activities in your job if that has caused your injury. Continuing on with regular activities can increase the severity of your injury, turning a mild to moderate case of tendon strain into a downward spiral of atrophic damage that may eventually severely impact your life. Also, trying to 'work around' your injury will eventually give rise to over-compensation injuries in other areas of your body.
  • Use a Cold Compress or Ice Pack - Cold is very effective at reducing pain and inflammation - use at the onset of the injury and during flareups.

  • Stretching - Stretching your joint in PT and at home will help you to regain your range of motion much faster than not stretching at all. How Scar Tissue Affects Tendon Recovery and Rehabilitation

Tendons, ligaments, muscle and other soft tissue in the joint are all meant to be soft and flexible, ready to work and move small to extreme forces in everyday activities. When I say extreme force, I mean try to imagine the amount of tension that is put on your ankles, hips and knees when running or climbing stairs - even when you are just walking even, let alone running. Shoulders and elbows often deal with heavy or repetitive forces at work if you are a tradesman, garden or work in an assembly line. Try to imagine the amount of force that your arm puts on your elbow and shoulder when you are just trying to throw a football or lift something heavy.

Scar tissue grows in damaged tissue when it tries to heal; little tiny band-aids that overlap each other to bind tiny tissue tears together. With this added scar tissue, muscles & tendons & ligaments become rigid, less flexible and unable to handle the forces that it once could. If scar tissue is found in a tendon or muscle, this can impede joint movement and/or cause pressure points - increasing the risk of further soft tissue injuries and inflammation. If you're suffering with scar tissue now you may feel the effects with stiffness, tightness, weakness and tiredness in your joint and muscles.

Scar tissue can form fast to bring together the edges of a tear, but working fast doesn't mean that the job's done right. When scar tissue forms it doesn't come together as neatly as regular (healthy) tissue would. Scar tissue fibers will lay down over top of your tear in a cluttered, messy and jumbled up way.

Scar tissue will form fast to deal with a soft tissue injury, and this scar tissue will attach to EVERYTHING in the area, including the surrounding healthy tissue as well. This can result in a fusing together of soft tissue that shouldn't be fused together (muscles,tendons,ligaments), and this will cause extreme pain when you move your leg - it is literally ripping scar tissue. This is why PT is often painful - the therapist stretches the joint, forcing the scar tissue bonds to break so you can regain your range of motion.

Scar tissue can become a major problem and cause your injury to become chronic - taking months or even YEARS to completely heal.

Scar tissue is something that will be present in and around your tendons and muscles after a soft tissue injury. The growth of scar tissue is ultimately what causes stiffening in the joint, restricting movement and flexibility. If you undergo surgery, you will find that scar tissue growth is something that cannot be avoided during your recovery. Your surgeon will determine if the anticipated outcome from surgery will be successful, despite the buildup of scar tissue that you will develop as a result of the surgery. Overall, the surgeon may be able to remove a lot of the initial buildup of scar tissue around the injury and in doing so, view a positive outcome from the surgery.

Unfortunately, scar tissue may plague you for weeks, months and maybe even years after your surgery, depending on your level of activity and the amount of conservative treatments you have done during your rehabilitation. Scar tissue is a major problem, especially when it comes to re-injury risk.

What About Surgery?

Supports and Exercises

Your doctor may encourage you to use a cuff or brace. The brace or cuff typically adds light compression to the area to support the tendons. Most physicians caution you in not wearing a brace too frequently, as there is often restricted blood flow and the soft tissue is then not allowed the time to heal. In some cases, prolonged use of a brace may cause increased secondary conditons such as pinched nerves and interruption to the blood flow to the joint.



Restoring Flexibility and Strength

Visits to the Physical Therapist is a very helpful way to decrease pain in soft tissue, restore strength to atrophied muscles and improve strength and mobility. Your PT will start you on a program that will be tailored for your tendon injury. Once your pain starts to reduce, a physical therapist will also set up an individualized strengthening and stretching exercise program for you to perform at home or in the gym.

This will be based on your needs and abilities, and will help you return to performing your normal routines.

A trained physical therapist will help to ensure your rehabilitation process is effective. Okay, So I Have a Damaged Tendon. What Should I Do Now?

If you have tendinitis - or any type of tendon damage really - rest is highly recommended. Avoid activities that cause pain or may have caused the inflammation and begin cold compression treatments as soon as possible.

During the healing process, scar tissue builds in areas where the tendons, muscles and tendon sheath has been damaged. This scar tissue can cause the tendon to thicken, decreasing the area of movement in tight spaces (such as the subacromial spaceor wrist, for example) and, in such cases, increasing the chance of impingement.

In addition, scar tissue can bind tendons and muscles and ligaments - and anything else together and this is not a good thing! Inflexible scar tissue limits movement of these tendons and ligaments that were once elastic; what it winds up doing is really freezes up your range of motion, and it often will continue to worsen if left untreated. At the very least, scar tissue leaves your tendon weaker than a healthy tendon, leaving your joint at greater risk of further deterioration and possible tearing.

Once the inflammation from your damaged tendon has been reduced through use of a Cold Compress or Ice Pack, nourishing and strengthening of the tendons and muscles in the area is recommended. Stage 1: If the injury is very recent - use a good quality cold pack and Arnica Pain Relief Cream.

Not only will you use your cold pack for the first 48 to 72 hours after the injury, but you should also use it during other stages of the healing process. Some examples include...

  • If you have a job that requires you to place some demands on your injured tendon. If that is the case, use cold immediately following work and if the opportunity allows - use it for 15 to 20 minutes during lunch or another break time.
  • If you are in a post-operative situation where cold compression would be utilized heavily for at least the first few weeks following the procedure. Similarly, if you are scheduled for upcoming surgery, then you will need a Cold Compress or Ice Pack to use.
